Show Notes

A breast lift is one of the most common plastic surgery procedure year after year, regardless of whether or not an implant is used.  Using an implant can change the operation (and results) significantly.In this show, I discuss the how's, why's - and the why not's - of breast lift surgery.If you or someone you care about is interested in breast lift surgery, this episode is a "don't miss"!

00:00:13.500 --> 00:00:16.350

Jason: One of the most popular

procedures year over year in my

00:00:16.350 --> 00:00:18.050

office is a breast lift with implants.

00:00:18.470 --> 00:00:20.820

Most of my patients are

kind of in my demographic.

00:00:20.820 --> 00:00:22.783

They're women that are done having kids.

00:00:23.218 --> 00:00:26.455

Kind of want to improve and

get back the shapes that they

00:00:26.455 --> 00:00:29.585

had before kids stole that.

00:00:29.875 --> 00:00:34.095

That typically means lifting the

breast back up on the chest, reshaping

00:00:34.095 --> 00:00:36.675

it, and adding a little bit more

volume in the shape of an implant.

00:00:36.675 --> 00:00:42.655

I want to talk a little bit about that

procedure, what it is, what it isn't.

00:00:43.115 --> 00:00:46.985

recovery time off of work and

all of that to give you a good

00:00:47.035 --> 00:00:48.765

understanding of what that is.

00:00:49.165 --> 00:00:52.575

So breast lift with

implants, what are the goals?

00:00:52.735 --> 00:00:57.515

Well, the goals of a breast lift are to

improve the nipple position on your chest.

00:00:57.515 --> 00:01:02.095

We want to get that nipple back up above

the breast crease to a more youthful spot.

00:01:02.505 --> 00:01:08.335

We want to reshape the breast around

that nipple that's now in a new spot.

00:01:09.885 --> 00:01:14.510

We want to reduce areola that

get large with pregnancy.

00:01:14.510 --> 00:01:17.360

That's one of the things when we're

talking about lifts and implants,

00:01:17.360 --> 00:01:20.490

that's typically one of the first

questions like are you going to

00:01:20.490 --> 00:01:21.880

make my nipples a little smaller?

00:01:22.120 --> 00:01:26.280

And the answer to that is yes,

is that is a part of a lift

00:01:26.280 --> 00:01:27.330

with or without an implant.

00:01:28.565 --> 00:01:33.415

And then lastly is to remove excess

breast skin that kind of sits between

00:01:33.505 --> 00:01:35.365

the nipple and the breast crease.

00:01:35.735 --> 00:01:38.335

That skin and breast tissue

that can get stretched out

00:01:38.335 --> 00:01:40.275

over time and drop and descend.

00:01:40.285 --> 00:01:41.575

So we want to remove that.

00:01:41.575 --> 00:01:46.655

Just as important though to know what

a breast lift with an implant does do,

00:01:47.230 --> 00:01:51.040

Let's talk about what a breast lift

doesn't do because it's important to

00:01:51.040 --> 00:01:54.810

understand that and we spend a lot

of time in our consultations talking

00:01:54.820 --> 00:01:59.150

about what doesn't happen so that we

can make plans if that's something

00:01:59.150 --> 00:02:01.470

that you're interested in to improve.

00:02:01.470 --> 00:02:04.640

So that we can make plans if one

of the things that it doesn't do.

00:02:05.145 --> 00:02:08.775

is one of the things that you're after,

that we can make plans to accomplish

00:02:08.775 --> 00:02:12.745

those goals because you don't want

to go through all of this surgery and

00:02:12.745 --> 00:02:15.545

then look and say, well, I thought

that it did this and it doesn't.

00:02:15.985 --> 00:02:17.725

So what does a breast lift not do?

00:02:17.935 --> 00:02:21.385

Big thing is a breast lift

doesn't improve hollowness in

00:02:21.385 --> 00:02:23.235

the upper pole of the breast.

00:02:23.545 --> 00:02:30.045

Most women Will require some sort of

volume augmentation, either in the

00:02:30.045 --> 00:02:34.655

form of an implant or in the form

of fat grafting to be able to fill

00:02:34.655 --> 00:02:37.475

out that upper pole of the breast.

00:02:37.525 --> 00:02:42.425

Upper pole hollowness isn't corrected

with a breast lift by itself.

00:02:42.485 --> 00:02:43.635

You have to have an implant for that.

00:02:44.280 --> 00:02:49.670

The second thing that it doesn't do is a

breast lift won't improve stretch marks.

00:02:49.970 --> 00:02:52.640

If you've got stretch marks in

your breast, that is the result

00:02:52.640 --> 00:02:55.640

of damage to the dermis or the

underlying part of the skin.

00:02:55.640 --> 00:02:57.710

That will not get better

with a breast lift.

00:02:57.710 --> 00:03:01.140

And if we add an implant, it may

actually make that a little bit worse.

00:03:01.170 --> 00:03:03.880

So that's something that

you have to be ready for.

00:03:03.880 --> 00:03:06.290

The other thing that it

won't fix is side boob.

00:03:06.680 --> 00:03:12.190

If you've got breast tissue or fat

that's accumulated under your arms.

00:03:12.190 --> 00:03:14.360

Typically, a lift will help improve that.

00:03:14.500 --> 00:03:18.840

Oftentimes, we have to add liposuction

or some other treatment to your

00:03:19.040 --> 00:03:24.070

chest to help improve that side boob

and get rid of that when we do our

00:03:24.070 --> 00:03:26.570

breast lift or lifting implants.

00:03:27.840 --> 00:03:31.920

Likewise, poor skin quality

, kind of loose, saggy skin.

00:03:32.225 --> 00:03:36.645

Is actually something that we talk a

lot about because that is largely what

00:03:36.695 --> 00:03:38.715

causes you to need a breast implant.

00:03:38.925 --> 00:03:42.335

We don't change that skin

or tissue quality over time.

00:03:43.055 --> 00:03:45.455

And so that's something we have

to talk about how we're going to

00:03:45.465 --> 00:03:50.255

handle that both in our initial

surgery and planning for later.

00:03:50.255 --> 00:03:56.655

And then lastly, if you're someone who

has a long torso and your breasts sit

00:03:56.675 --> 00:04:01.525

down at the bottom part of your torso,

your breast crease is fairly low.

00:04:01.525 --> 00:04:02.675

That will not change.

00:04:02.675 --> 00:04:04.855

Your breasts will still

sit low on your chest.

00:04:04.855 --> 00:04:07.515

You'll still have a long distance

between your collarbone and

00:04:07.515 --> 00:04:08.705

the top part of your breast.

00:04:09.115 --> 00:04:10.535

Even if we add an implant.

00:04:10.535 --> 00:04:14.445

Your breast kind of has a footprint

like your foot does, and we

00:04:14.445 --> 00:04:16.195

can't change that footprint.

00:04:16.195 --> 00:04:20.855

So if you're a woman who has breasts that

sit low on our chest, they're still going

00:04:20.855 --> 00:04:22.455

to sit low on your chest when we're done.

00:04:24.215 --> 00:04:29.135

So now that we know what a breast

lift accomplishes, what a breast lift

00:04:29.145 --> 00:04:33.045

doesn't accomplish, let's talk about

how the procedure is actually done.

00:04:33.465 --> 00:04:38.335

With breast lifts, there are three

incision types that you'll see if

00:04:38.335 --> 00:04:39.915

you're digging around on the internet.

00:04:40.125 --> 00:04:42.275

You can call them the

small, medium, and large.

00:04:42.495 --> 00:04:45.885

There is the donut or

just around the areola.

00:04:46.325 --> 00:04:50.425

incision, which really is a

bad incision for a breast lift.

00:04:50.425 --> 00:04:53.375

It doesn't accomplish much

in the way of lifting.

00:04:53.665 --> 00:04:57.745

It's more used when we're using an

implant to just tighten up a little bit

00:04:57.745 --> 00:05:03.115

of skin around that area, maybe move it

a centimeter or two, but not getting a

00:05:03.115 --> 00:05:05.135

whole lot of lift out of that incision.

00:05:05.135 --> 00:05:08.725

Then you move up to

the medium or lollipop.

00:05:09.050 --> 00:05:11.730

style incision where we

add a vertical incision.

00:05:11.740 --> 00:05:16.720

So around the areola and then down from

the areola to the breast crease, that is

00:05:16.720 --> 00:05:23.120

the second type of incision that is for

women who need a little bit more lift

00:05:23.120 --> 00:05:25.570

to recenter that nipple on your chest.

00:05:25.570 --> 00:05:30.270

And then the third incision and probably

the one that people are the most familiar

00:05:30.270 --> 00:05:35.250

with is the teddy bear or ship's anchor

incision where we take that lollipop

00:05:35.260 --> 00:05:39.930

incision and then add an incision in

the breast crease of variable lengths

00:05:39.930 --> 00:05:45.230

depending on how much extra tissue there

is in that lower part of the breast

00:05:45.230 --> 00:05:47.300

between the nipple and the breast crease.

00:05:47.300 --> 00:05:50.340

Those incisions typically heal well.

00:05:50.860 --> 00:05:54.890

The incision that everybody seems to be

worried about is that vertical incision.

00:05:55.400 --> 00:06:01.280

And those heal fairly well, provided

there's not a lot of tension on them.

00:06:01.280 --> 00:06:04.630

Tension is really the enemy

of good wound healing.

00:06:05.070 --> 00:06:11.790

And that's why during our consultation

and When we're talking about adding

00:06:11.820 --> 00:06:17.600

a lift and implants, that procedure

can be done in a single stage, but

00:06:17.600 --> 00:06:19.360

we're kind of walking a tightrope.

00:06:19.530 --> 00:06:23.000

You're walking a tightrope between

too little tension and breasts

00:06:23.000 --> 00:06:26.910

being sag, still being a little bit

saggy, too much tension and scars

00:06:26.910 --> 00:06:29.300

getting wide or worse coming apart.

00:06:29.300 --> 00:06:33.660

And so we want to navigate

that balance beam just right.

00:06:34.455 --> 00:06:39.855

And plan for later, if things are

a little bit loose, to revise that

00:06:40.375 --> 00:06:44.475

scar, do a little bit of tightening

later once everything is healed.

00:06:45.275 --> 00:06:47.925

Because falling off the other side

of that balance beam where things

00:06:47.925 --> 00:06:52.145

are too tight, you can get into

some really long term problems.

00:06:52.205 --> 00:06:55.945

The other option there is doing

it in two stages, which a lot of

00:06:55.945 --> 00:06:59.125

women don't want to do, but we talk

about that during consultations

00:06:59.165 --> 00:07:01.205

because that is another option.

00:07:02.865 --> 00:07:06.945

Surgery for breast lift with an

implant or breast lift alone takes

00:07:06.945 --> 00:07:08.525

between three and four hours.

00:07:08.975 --> 00:07:11.965

In my practice, I'm operating by myself.

00:07:11.965 --> 00:07:15.785

I don't have somebody cutting and

sewing and, you know, I'm doing one

00:07:15.785 --> 00:07:16.835

side and they're doing the other.

00:07:16.835 --> 00:07:17.475

It's all me.

00:07:17.695 --> 00:07:19.845

So it takes between three or four hours.

00:07:20.375 --> 00:07:21.815

You're at home the same day.

00:07:21.855 --> 00:07:25.815

We typically don't use

drains for these anymore.

00:07:26.700 --> 00:07:27.610

nObody wants them.

00:07:27.840 --> 00:07:30.660

And drain tubes are typically

not something that we

00:07:30.660 --> 00:07:32.390

use for these procedures.

00:07:32.390 --> 00:07:34.740

I operate here in our

private surgery center.

00:07:34.740 --> 00:07:38.410

Once surgery is over we, you

spend about 30 minutes in the

00:07:38.410 --> 00:07:41.420

recovery room, go home the same day.

00:07:41.830 --> 00:07:43.910

I tell patients, you're

going to lose the morning.

00:07:44.440 --> 00:07:48.950

You're You get a little bit of medication

before surgery, you, last thing you really

00:07:48.950 --> 00:07:52.160

remember is talking to me and having

me mark things right before surgery.

00:07:52.310 --> 00:07:54.180

Next thing you know, you

wake up on your couch.

00:07:54.470 --> 00:07:59.330

For women that have had other procedures,

hospitals and other surgery centers

00:07:59.870 --> 00:08:02.690

the surgery day is fairly pleasant.

00:08:02.700 --> 00:08:05.120

You don't really remember a whole lot.

00:08:06.490 --> 00:08:11.050

Recovery in plastic surgery

is very surgeon dependent.

00:08:11.090 --> 00:08:15.620

There's a lot of, what works for

me that surgeons get into during

00:08:15.620 --> 00:08:17.520

their post op recovery time.

00:08:18.080 --> 00:08:22.500

For my patients with a breast lift,

breast lift with implants, there's

00:08:23.115 --> 00:08:25.135

Surgical tape that covers your incisions.

00:08:25.175 --> 00:08:27.205

There's no stitches that have to come out.

00:08:27.545 --> 00:08:29.005

You can shower the next morning.

00:08:29.005 --> 00:08:31.445

Actually, I encourage you to shower

the next morning because you want to

00:08:31.445 --> 00:08:35.165

be clean, but you feel more human once

you've kind of been able to take a

00:08:35.165 --> 00:08:39.295

shower, wash some of the funk off and

freshen up a little bit that next day.

00:08:40.420 --> 00:08:44.800

There are no restrictions in terms

of what you can and can't do from

00:08:44.800 --> 00:08:46.440

a range of motion standpoint.

00:08:46.490 --> 00:08:48.950

A lot of women ask me if

they can wash their hair the

00:08:48.950 --> 00:08:50.480

next day, the answer is yes.

00:08:50.670 --> 00:08:53.950

What I don't want you doing is

lifting anything, is doing any

00:08:53.950 --> 00:08:58.310

movement where your chest muscles or

arm muscles are loaded, and you're

00:08:58.310 --> 00:09:01.770

trying to reach up, you're lifting

something heavy up above your head.

00:09:02.535 --> 00:09:05.525

or trying to pick things up off

the floor, that includes kids.

00:09:06.085 --> 00:09:11.085

So for moms who are having this, who

have young kids, you either want help, or

00:09:11.085 --> 00:09:15.615

you want to let your children climb into

your lap, stand up with them, instead

00:09:15.615 --> 00:09:17.475

of trying to pick them up off the floor.

00:09:20.165 --> 00:09:23.895

Things are going to look weird

that first week after surgery.

00:09:23.945 --> 00:09:25.425

There's no two ways around it.

00:09:25.925 --> 00:09:27.045

Things swell.

00:09:27.325 --> 00:09:27.945

Things swell.

00:09:28.235 --> 00:09:30.585

You may have a teeny bit of

bruising if we're doing some

00:09:30.585 --> 00:09:34.205

liposuction but you, they look weird.

00:09:34.555 --> 00:09:37.165

If we're using implants, the top

part of your breast can swell.

00:09:37.165 --> 00:09:39.835

It can make your nipples look like

they're pointed down a little bit.

00:09:39.835 --> 00:09:41.305

All of that is normal.

00:09:41.785 --> 00:09:44.565

Obviously if it's something

you're concerned about, I want my

00:09:44.565 --> 00:09:45.765

patients to let me know about it.

00:09:46.005 --> 00:09:48.045

If you're somebody else's

patient, let them know about it.

00:09:48.045 --> 00:09:56.525

that is relatively normal right after

surgery in terms of working and return

00:09:56.525 --> 00:09:58.915

to work kind of depends on what you do.

00:09:59.225 --> 00:10:03.245

If you have a desk job, you work at

a computer, you can work remotely.

00:10:03.735 --> 00:10:07.605

Typically after a week, , you're

not taking narcotic medication.

00:10:07.615 --> 00:10:10.925

You're able to kind of go

back to your normal routine.

00:10:11.195 --> 00:10:14.320

If you've got a very physical

job, I recommend people

00:10:14.380 --> 00:10:15.510

taking about two weeks off.

00:10:15.510 --> 00:10:20.940

You can always go back early if you feel

great and you want to but depending on the

00:10:20.940 --> 00:10:23.410

nature of your work, one to two weeks off.

00:10:24.280 --> 00:10:27.070

Now, I don't want anybody

doing any exercise.

00:10:27.080 --> 00:10:31.870

So lifting more than about 10 pounds or

bouncing, those are the two restrictions,

00:10:31.880 --> 00:10:34.050

lifting and bouncing for six weeks.

00:10:34.240 --> 00:10:39.790

Really want those implants to heal before

we start stressing those implants at all.

00:10:40.830 --> 00:10:46.210

There's a I did a podcast back in a couple

of months ago about sex after surgery.

00:10:46.270 --> 00:10:47.460

You can listen to that one.

00:10:47.620 --> 00:10:48.830

That's always a question.

00:10:49.420 --> 00:10:51.270

The bouncing really comes in there.

00:10:52.030 --> 00:10:55.890

In terms of new clothes, wait

three to four months until you

00:10:55.890 --> 00:10:57.360

start going swimsuit shopping.

00:10:57.360 --> 00:10:59.670

You start going bra shopping,

things are going to be swollen.

00:10:59.900 --> 00:11:02.230

You don't want to buy a bunch of

new stuff when you're still swollen.

00:11:02.390 --> 00:11:03.730

Especially if it's expensive stuff.

00:11:04.100 --> 00:11:07.565

And then the swelling goes down

and your new stuff doesn't fit.

00:11:08.035 --> 00:11:13.115

So wait three to four months

before you start going shopping.

00:11:14.735 --> 00:11:17.555

Scar treatments are variable.

00:11:17.775 --> 00:11:20.655

Now that's kind of the next thing

is what do we do about scars?

00:11:21.535 --> 00:11:25.465

We can start topical scar treatments

as early as a week after surgery.

00:11:26.405 --> 00:11:28.485

aNd that's topical scar treatments.

00:11:28.795 --> 00:11:33.915

We recommend a silicone tape or

silicone gel over your scars.

00:11:34.035 --> 00:11:36.785

We can also start laser

treatments as early as a week.

00:11:36.815 --> 00:11:40.615

So as soon as our surgical tape comes

off, we can start treating scars.

00:11:40.965 --> 00:11:45.645

If you have had problems with scarring

in the past or just want to be super

00:11:45.645 --> 00:11:48.475

aggressive we can start that pretty early.

00:11:48.475 --> 00:11:53.065

So a common thing that comes up is,

why would I not want to do this?

00:11:53.615 --> 00:11:57.995

And this is really where we get

into the reason that you shouldn't

00:11:58.045 --> 00:12:00.515

have a lift and or implants.

00:12:01.125 --> 00:12:04.395

We talk in terms of relative

and absolute contraindications.

00:12:04.405 --> 00:12:09.345

So relative contraindications mean,

eh, probably not the greatest idea.

00:12:09.725 --> 00:12:15.500

You run the risk of either some

medical problems or a cosmetic

00:12:15.500 --> 00:12:17.100

outcome that's less than perfect.

00:12:18.120 --> 00:12:20.050

And then you've got absolute

contraindications, which

00:12:20.050 --> 00:12:22.990

means it's really not a good

idea to have surgery at all.

00:12:23.860 --> 00:12:27.500

So, relative contraindications

to a lift, certainly a lift with

00:12:27.500 --> 00:12:30.380

implants, is poor tissue quality.

00:12:31.040 --> 00:12:35.570

And, thin, saggy, sad breast tissue.

00:12:35.570 --> 00:12:38.230

Thin skin with lots of stretch marks.

00:12:38.730 --> 00:12:43.190

The chances that you are going

to need a revision later, no

00:12:43.190 --> 00:12:46.160

matter what we do, is pretty high.

00:12:46.420 --> 00:12:50.830

Like I said at the very beginning,

we can't treat poor quality tissue.

00:12:50.830 --> 00:12:54.125

If you've got, collagen that

has kind of melted away.

00:12:54.375 --> 00:12:58.095

If your skin is thin, there's nothing

that we're going to do for that.

00:12:59.215 --> 00:13:02.615

And so going into that, if that's,

if you're kind of in that camp, it

00:13:02.625 --> 00:13:03.975

doesn't mean we can't do surgery.

00:13:03.975 --> 00:13:05.765

It doesn't mean we can't

get you a good result.

00:13:06.335 --> 00:13:11.465

It means that we have to be careful

and plan for the fact that your skin

00:13:11.485 --> 00:13:14.125

is going to relax after surgery.

00:13:14.135 --> 00:13:17.265

Once the swelling goes

away, it is going to relax.

00:13:17.585 --> 00:13:21.775

And so that means being conservative

in implant choices, not putting a

00:13:21.775 --> 00:13:24.285

huge implant in trying to stretch

things out because that's just

00:13:24.285 --> 00:13:28.335

going to make the problem worse

means not pulling things too tight.

00:13:28.875 --> 00:13:34.190

It means accepting that there's a

possibility that Six months a year down

00:13:34.190 --> 00:13:36.890

the road, we're gonna want to come back.

00:13:36.890 --> 00:13:40.070

That tissue will have sagged a little

bit and we're gonna want to come

00:13:40.070 --> 00:13:45.050

back and tighten that up a little bit

more to get the result that we want.

00:13:45.230 --> 00:13:47.060

So does it mean no surgery?

00:13:47.065 --> 00:13:47.330

No.

00:13:47.330 --> 00:13:50.965

It just means we need to

understand that a second little

00:13:51.295 --> 00:13:53.635

tuck up may be needed at a year.

00:13:54.815 --> 00:13:56.675

If you have medical problems.

00:13:57.425 --> 00:13:59.085

that make anesthesia dangerous.

00:13:59.085 --> 00:14:01.575

And this is a relative

contraindication to everything.

00:14:01.855 --> 00:14:05.095

If you've got medical problems

that make anesthesia dangerous,

00:14:05.265 --> 00:14:06.385

we need to think about that.

00:14:06.385 --> 00:14:09.015

This is surgery that you want,

not surgery that you need.

00:14:09.405 --> 00:14:13.685

And so we don't want to take medical

risks with your health just to

00:14:13.695 --> 00:14:15.455

give you better looking breasts

00:14:15.455 --> 00:14:16.205

smoking.

00:14:16.385 --> 00:14:23.536

And this is one that comes up a lot is it

causes problems with microcirculation and

00:14:23.586 --> 00:14:27.836

when we're doing a breast lift, certainly

when we're doing a breast lift with

00:14:27.946 --> 00:14:36.266

implants, nicotine impairs the ability

for your body to heal those incisions that

00:14:36.266 --> 00:14:38.986

we need to heal to get you a nice result.

00:14:39.466 --> 00:14:45.436

A lift with implants is typically

in my book, a no no for having

00:14:46.051 --> 00:14:47.651

cosmetic breast surgery like this.

00:14:47.651 --> 00:14:49.201

You have to be nicotine free.

00:14:49.231 --> 00:14:51.231

We want to optimize your results.

00:14:51.631 --> 00:14:57.251

So six weeks before, six weeks

after surgery, no nicotine at all.

00:14:57.251 --> 00:15:01.641

That includes nicotine gum,

nicotine pouches, things like

00:15:01.641 --> 00:15:05.451

that., Cigarette smoking, nicotine

is a no for this kind of surgery.

00:15:05.451 --> 00:15:09.891

So we'll wait until you're T we

can get you off before we proceed.

00:15:10.931 --> 00:15:12.201

In a new relative country.

00:15:12.201 --> 00:15:17.291

And it's certainly something we have to

talk about is Ozempic and these Monjaro

00:15:17.291 --> 00:15:19.511

and Tizipatide and all these other things.

00:15:20.331 --> 00:15:23.421

And the reason that those are

problematic is not for healing

00:15:23.421 --> 00:15:26.331

necessarily, but is for anesthesia.

00:15:26.331 --> 00:15:28.171

They cause your gut to slow down.

00:15:28.211 --> 00:15:31.981

When our anesthetist are putting you

to sleep, your stomach has to be empty.

00:15:32.041 --> 00:15:35.721

Otherwise we run the risk of,

essentially vomiting with you asleep,

00:15:35.731 --> 00:15:40.261

which can get in your lungs and cause

really nasty inflammatory problems.

00:15:40.471 --> 00:15:42.301

It can land you in the

hospital and the ICU.

00:15:42.896 --> 00:15:47.076

Ozympic, we really need to stop those

things about six weeks before surgery.

00:15:47.166 --> 00:15:53.756

Let your gut kind of come back online,

start functioning properly so that we

00:15:53.766 --> 00:15:56.386

don't run the risk of anesthesia problems.

00:15:56.386 --> 00:15:59.726

Now that we've gotten through

the kind of contraindications for

00:15:59.726 --> 00:16:01.496

surgery, talk about some of the risks.

00:16:02.241 --> 00:16:06.131

Typically in cosmetic surgery,

the idea is that we want to get

00:16:06.131 --> 00:16:08.121

our complications down to zero.

00:16:08.131 --> 00:16:12.781

Now as surgeons, we understand

that can't happen as patients.

00:16:12.811 --> 00:16:16.951

You need to understand that there

is a chance you will have a problem

00:16:16.951 --> 00:16:22.171

after surgery, but we try and

minimize those as much as possible.

00:16:22.411 --> 00:16:26.291

So what are the bad ones?

00:16:27.456 --> 00:16:28.706

What's the worst thing that can happen?

00:16:28.776 --> 00:16:32.256

It's kind of how my parents brought me up

with think about what's the worst thing

00:16:32.266 --> 00:16:33.876

that can happen before you do anything.

00:16:34.916 --> 00:16:39.316

The worst thing that can happen is that

we do a breast lift with implants and

00:16:39.316 --> 00:16:41.836

we either have dead tissue, things die.

00:16:41.936 --> 00:16:46.176

We get too aggressive and stretch

things too far and they die.

00:16:47.296 --> 00:16:53.766

oR we get an infection and end up losing

implants, get infected, exposed, and we

00:16:53.766 --> 00:16:55.656

have to take them out and start over.

00:16:56.156 --> 00:17:00.586

And so fortunately those

complications are really rare.

00:17:00.666 --> 00:17:02.956

They do happen, but they're really rare.

00:17:03.796 --> 00:17:08.756

And they're more common in

patients who have healing problems.

00:17:08.756 --> 00:17:12.756

So things like diabetes, patients who

smoke, which is one of the reasons that.

00:17:13.606 --> 00:17:18.166

Smoking is kind of a foreboding thing

around the time of surgery, or patients

00:17:18.166 --> 00:17:20.386

who are on some immune modulators.

00:17:20.396 --> 00:17:25.626

So taking medications for

rheumatoid arthritis or ulcerative

00:17:25.676 --> 00:17:28.526

colitis or inflammatory bowel

disease or things like that.

00:17:28.846 --> 00:17:31.816

Those are things that we have to

really be careful with because they

00:17:31.816 --> 00:17:35.766

can cause healing problems that can

result in some really disastrous

00:17:35.766 --> 00:17:37.896

complications from a cosmetic standpoint.

00:17:38.296 --> 00:17:40.916

Blood clots around the

implants, a hematoma.

00:17:41.236 --> 00:17:43.366

Those are really also very rare.

00:17:43.366 --> 00:17:45.976

Fortunately, those require a trip

to the operating room to drain.

00:17:46.896 --> 00:17:49.666

The more kind of annoying problems.

00:17:49.986 --> 00:17:52.756

that can happen after

surgery is sensory loss.

00:17:52.946 --> 00:17:55.516

Around the nipple sensory loss can happen.

00:17:55.686 --> 00:18:00.206

Typically, the likelihood of that

happening goes up the further

00:18:00.206 --> 00:18:01.316

we have to move the nipple.

00:18:01.376 --> 00:18:05.556

So the more we have to do to kind

of move the nipple around and get

00:18:05.556 --> 00:18:07.056

it re centered back on your chest.

00:18:07.056 --> 00:18:09.646

the more likelihood you are

to have some sensory changes.

00:18:10.026 --> 00:18:11.566

Now we don't take the nipple off.

00:18:11.566 --> 00:18:12.716

That's another common question.

00:18:12.716 --> 00:18:14.746

People do you take the nipple

off and put it back on?

00:18:15.026 --> 00:18:16.106

It stays attached.

00:18:16.106 --> 00:18:19.466

And that's why what we really have

to be careful with is that kind of

00:18:19.516 --> 00:18:22.676

keeping the nipple on that leash

to move it around on the inside.

00:18:23.096 --> 00:18:25.536

That's where some of the

nerves and blood vessels live.

00:18:25.816 --> 00:18:30.646

And the smaller we make that

leash, the more, chances of

00:18:30.646 --> 00:18:32.956

problems we're going to have.

00:18:33.016 --> 00:18:36.286

And some patients with, you know, we

have to make that leash really long.

00:18:36.816 --> 00:18:38.036

It can be a real issue.

00:18:38.036 --> 00:18:39.936

And we talk about that before surgery.

00:18:40.736 --> 00:18:43.996

Scar healing is another one that

we kind of touched on earlier.

00:18:44.366 --> 00:18:46.396

And scar healing is one of those things.

00:18:46.786 --> 00:18:48.996

If we want to be aggressive,

we can start early.

00:18:49.286 --> 00:18:53.796

Otherwise, we watch mother nature

take a course and can course correct

00:18:53.796 --> 00:18:55.536

if we see things going the wrong way.

00:18:55.916 --> 00:19:00.286

That is breast lift and

implant surgery in a nutshell.

00:19:01.426 --> 00:19:06.116

I tried to be as thorough about this and

let you know what to expect as possible.

00:19:06.446 --> 00:19:08.116

Obviously, with medicine.

00:19:08.116 --> 00:19:12.186

surgery, cosmetic surgery,

everybody's different, everybody's

00:19:12.196 --> 00:19:13.356

plans a little different.

00:19:13.886 --> 00:19:17.976

And if this is something that you are

interested in, we will go through all of

00:19:17.976 --> 00:19:21.536

this in detail at your consultation, but

wanted to give you an overview of things.

00:19:21.846 --> 00:19:25.436

So you know what you're getting into

before you go in for your consultation.

00:19:25.826 --> 00:19:29.356

If you have any questions, as always,

shoot me a DM, shoot me an email.

00:19:29.576 --> 00:19:30.446

Thanks for listening.

00:19:30.456 --> 00:19:31.586

And we'll talk to you soon.
